Frank Dewayne Forman

Frank Dewayne Forman, age 55 of Manchester, KY, left this life on Wednesday, November 30, 2016. Frank was born in Hamilton, Ohio on June 14, 1961 to Willis Frank Forman and Judy Forman. Frank was a hardworking man, with a kind heart. His daughters meant the world to him. He loved his family very much and will be greatly missed.

Frank is survived by his parents, Frank (Judy) Forman; his two daughters, Amber Nicole Forman and Crystal Rose Forman, both of Booneville, KY; one brother, Daryle (Jacqueline) Forman of Middletown, OH; his niece Misty (Shane) Miller; his nephew Daryle W. Forman Jr. of Middletown, OH; two great nieces, Alivia Sue Miller and Kerrie Lynn Miller; and many other family members, aunts, uncles, and cousins. Frank was preceded in death by his grandparents Homer and Glada Forman, and Eli and Mae Collins; and a very special aunt Evelyn Crouch Lunsford (Forman).

Funeral service will be held at Brown Dawson Flick Funeral Home, 1350 Millville Ave, Hamilton, Ohio, on Tuesday, December 6, 2016 at 1:30 PM with Pastor Arnold Brewer officiating. Burial will follow at Somerville Cemetery. Visitation will be held on Tuesday, December 6, 2016 from 12:30 PM to 1:30 PM at Brown Dawson Flick Funeral Home.
